Before You Begin
For new breast patients:
Have you had a breast biopsy or surgery performed at a facility other than a Methodist or Baptist facility? OR have you had breast pathology performed at any place other than Memphis Pathology Group or Pathology Group of the Midsouth?
If you answered YES to any of the above, you will FIRST need to fill out the online Authorization form so we can receive your medical records before you are seen.

*PLEASE NOTE* This packet must be filled out if you are scheduled for a Physician Office visit. You are not required to fill this out if you are ONLY coming in for a Screening or Diagnostic Mammogram.
